The defeat was the Australian's eighth against Medvedev, who recorded his 50th top 10 win in dispatching world No.7 De Minaur.
More than 32 points went beyond nine shots across the match and ultimately it was Medvedev that proved the stronger of the two as De Minaur struggled to reach his peak.
Medvedev hit 11 more winners than the Australian, 27 to 16, and committed two fewer unforced errors, winning 15 of his 16 first-serve points to claim victory and advance to the semi-finals.
"I was super tired and I knew against Alex, like against Learner, we would have long points. I think in the third or fourth game we had a couple and I thought it was going to be a long day, but I am happy with the way I played," Medvedev said.
"I was really clutch in the important moments and hitting the ball great and putting him under pressure. The second set was tight but I found the best at the end. I am really happy with the level."
De Minaur leaves Shanghai looking good for a spot at the Nitto ATP Finals at the end of the year. He is 640 points clear of 10th-placed Felix Auger-Aliassime, who is the first player outside the top eight cut because ninth-placed Jack Draper is out for the rest of the year with injury.
It will be De Minaur's second consecutive appearance at the Finals, should he make it.
Medvedev, meanwhile, moves through to chase his first ATP Tour title since Rome in May 2023 and he will next meet Arthur Rinderknech in the last four, who beat Auger-Aliassime in the other last-eight clash of the day.
Medvedev reached the semi-finals at Indian Wells earlier this year, but lost to Holger Rune. He will be hoping for better in China, where he has a chance to add to the Shanghai Masters title he won in 2019.
