The 23-year-old Italian won 6-3 6-3 in just 69 minutes, as No.1 seed Zverev missed out on reaching another showpiece in his native Germany.
The first set was straightforward for Cobolli, who converted his only break point on the way to taking the lead.
Cobolli continued that momentum into the second set, breaking Zverev's serve in the first game and putting the German under immense pressure.
The Italian converted another break point to go 5-2 up, and although Zverev got one break back, he only delayed the inevitable as the world No.13 clinched victory with his first match point.
Cobolli will meet Ben Shelton in the showpiece, after last year's finalist secured a 6-3 6-4 win over qualifier Alex Molcan.
