Luke O'Nien's short pass inside his own box was intercepted by Nick Woltemade, who teed up Anthony Gordon to round his defender and drill a low shot into the bottom-left corner.
Chemsdine Talbi forced Ramsdale into a smart stop at the other end, while Sven Botman struck the post with a header just before the break.
Sunderland started the second half brightly, and Talbi dragged Sunderland level 57 minutes in, turning in the rebound after Habib Diarra's close-range effort was cleared on the line.
