The Hammers winger continued his fine recent form, with his deflected 95th-minute strike proving the difference at the Pirelli Stadium.
Nuno Espirito Santo's side survived a scare when referee Lewis Smith waved away a penalty appeal after Jake Beesley went down under Konstantinos Mavropanos' challenge.
West Ham improved in the second half, with Callum Wilson and Soungoutou Magassa denied by Bradley Collins, as the tie went into extra time.
Yet, the Burton goalkeeper was helpless five minutes into the first additional period, when Summerville embarked on a fine individual run from the left flank, before his 20-yard drive flashed in via a slight deflection off Terence Vancooten.
The Premier League side was reduced to 10 men in the 101st minute, with Freddie Potts shown a straight red card for a rash challenge on Julian Larsson.
But, despite facing late pressure with Kyran Lofthouse and Kain Adom going close for Burton, the Premier League strugglers held out to prevail.
