The 24-year-old was 4-6 6-3 4-1 down before withdrawing from the match, while he also received treatment at the end of the second set on a knee injury.
This was Draper's first match of the European clay swing and his first overall since losing to Reilly Opelka at last month's Miami Open.
The British No.2 was out for six months from last year's US Open because of an arm injury, making his return at the Dubai Tennis Championships in February.
Draper has won just four of his eight matches since returning, including a victory over Novak Djokovic as the Brit reached the last eight at Indian Wells, a title he won in 2025.
He is next scheduled to feature at the Madrid Open, an ATP Masters 1000 event, where he reached the final last year before losing in three sets to Casper Ruud.
