De Minaur, the No.3 seed, was expected to ease into the quarter-finals, but the signs of trouble were there in his previous match when he struggled through the first set against Sebastian Ofner.
And indeed Medjedovic proved to be no pushover as well, and dictated much of the play on the red clay.
The Serbian qualifier showcased impressive composure and shot-making, matching De Minaur’s trademark speed and even turning defence into attack with striking efficiency.
De Minaur struggled to gain a foothold in the rallies. Medjedovic broke serve early in both sets and maintained control, never allowing the Australian to build momentum.
Despite flashes of his usual counter-punching brilliance, De Minaur couldn’t convert key opportunities, particularly on return games where Medjedovic held firm under pressure.
The loss continues a mixed clay-court swing for the Australian.
For Medjedovic, it marks another significant scalp and underlines his growing confidence on the ATP Tour.
De Minaur's attention now turns to regaining rhythm ahead of the bigger European clay events, where Australia will hope its No.1 can quickly bounce back.
